Key Responsibilities:

  • Conduct thorough assessments of patients' psychosocial conditions, identifying strengths and challenges that may influence treatment outcomes.
  • Work collaboratively with patients and the healthcare team to develop effective interventions aimed at achieving rehabilitation and treatment goals.
  • Implement educational programs and utilize established social work methodologies to address barriers to care and enhance patient compliance.
  • Provide ongoing support and counseling to patients to facilitate their adjustment to dialysis and improve their psychosocial well-being.
  • Assist patients in understanding their health conditions and navigating community resources for optimal treatment outcomes.
  • Facilitate discussions regarding advance directives and ensure patients and families are informed about their care options.
  • Engage in quality improvement initiatives by monitoring adherence indicators and reporting on patient outcomes.
  • Deliver tailored education to patients and families regarding psychosocial aspects of end-stage renal disease and available support services.
  • Train clinic staff on psychosocial topics to enhance the overall care environment.

Qualifications:

  • Master's degree in Social Work.
  • State licensure as required.

Experience and Skills:

  • 0 – 2 years of relevant experience in a clinical setting.
