PURPOSE AND SCOPE:

The role involves delivering psychosocial support to patients receiving treatment at our facility, including those undergoing in-center and home dialysis, as applicable. Utilizing established Social Work principles, the position aims to foster positive adaptation, rehabilitation, and enhanced quality of life for patients. Collaborating with the healthcare team, the social worker will educate and support staff in recognizing the emotional, psychological, and behavioral implications of Chronic Kidney Disease on patients and their families, ensuring comprehensive care. This position aligns with Fresenius Kidney Care's commitment to quality indicators and improvement activities, including those focused on patient satisfaction and quality of life. This is an entry-level role for a Master Social Worker.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Assessment and Care Planning: As part of the interdisciplinary team, evaluate patients' psychosocial conditions, strengths, and needs that may influence their rehabilitation and treatment outcomes. Engage in collaborative care planning with patients and healthcare professionals to identify effective interventions aimed at achieving rehabilitation goals and enhancing quality of life. Utilize patient education programs and established social work methodologies to address barriers and meet treatment objectives. Provide ongoing support to patients adjusting to dialysis, ensuring optimal psychosocial well-being. Offer educational and goal-oriented counseling for patients pursuing transplant options and assist with referrals to community resources. Quality Assurance: Deliver psychosocial interventions to address adherence issues and enhance quality outcomes for all patients, tailored to their acuity levels. Participate in monthly quality review meetings with the interdisciplinary team, reporting on adherence-related quality indicators. Patient Education: Assess patients' understanding of kidney disease and identify barriers to treatment adherence. Collaborate with patients, families, and the healthcare team to provide tailored education that considers individual learning styles and communication needs. Facilitate the transplant referral process and ensure ongoing education regarding psychosocial aspects of End Stage Renal Disease (ESRD) and available support services. Patient Admission and Continuity of Care: Review essential patient information and address immediate concerns upon admission. Understand the referral and admission processes to support scheduling and maximize adherence. Conduct interviews to identify root causes for discharge requests and communicate findings with operational leadership. Insurance and Financial Assistance: Act as a liaison with Insurance Coordinators to resolve insurance-related issues. Provide patients with information regarding payment options for dialysis, including federal and state programs. Staff Collaboration: Assist in the recruitment process for new personnel as requested. Work with administrative staff to maintain updated patient resource lists and provide training on psychosocial topics as needed. Participate in team meetings to discuss urgent patient matters. Compliance: Adhere to defined caseload guidelines based on state regulations. Additional Duties: Perform other related responsibilities as assigned.
