Position Overview:
As a Clinical Social Worker at Fresenius Medical Care, you will be instrumental in delivering psychosocial support to patients in our dialysis facilities. Your role will involve collaborating with the healthcare team to enhance patient rehabilitation, emotional well-being, and overall quality of life. You will also assist clinic personnel in recognizing the psychological and behavioral challenges associated with chronic kidney disease.

Key Responsibilities:
- Conduct comprehensive assessments of patients' psychosocial conditions, identifying strengths and challenges that may influence their treatment outcomes.
- Collaborate with patients and the healthcare team to devise effective interventions aimed at achieving rehabilitation and treatment objectives.
- Implement patient education initiatives and utilize established social work methodologies to address barriers and facilitate treatment adherence.
- Provide ongoing support and counseling to patients, tailored to their specific needs and circumstances.
- Assist patients in accessing community resources and vocational rehabilitation services to optimize their treatment experience.
- Facilitate discussions regarding advance directives and ensure patients and families are informed about their care options.
- Monitor and report on quality indicators related to patient adherence and overall well-being.
- Educate patients and families about psychosocial aspects of end-stage renal disease and available support services.
- Deliver training to clinic staff on relevant psychosocial topics as necessary.
Qualifications:
- Master's degree in Social Work.
- Valid state licensure as required.
Experience:
- 0 – 2 years of relevant experience in a healthcare setting.
Work Environment:
