Social Work Specialist

PURPOSE AND SCOPE:
The Master Social Worker is responsible for delivering psychosocial support services to patients receiving treatment at our facility, including those undergoing in-center and home dialysis, when applicable. Utilizing established Social Work principles and methodologies, this role collaborates with the healthcare team to foster positive adaptation, rehabilitation, and enhanced quality of life for patients. In partnership with the interdisciplinary team, the Master Social Worker educates and guides staff in recognizing the emotional, psychological, and behavioral effects of Chronic Kidney Disease on patients and their families, ensuring a holistic approach to patient care. This position aligns with Fresenius Kidney Care's commitment to Quality Indicators, Outcomes, and Quality Assessment and Improvement (QAI) initiatives, actively participating in efforts to enhance patient satisfaction and quality of life. This is an entry-level position for a Master Social Worker.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment and Care Planning: Conduct comprehensive assessments of patients' psychosocial conditions, identifying strengths and challenges that may influence rehabilitation and treatment success. Collaborate with patients and the healthcare team to formulate effective care plans aimed at achieving rehabilitation goals and improving overall quality of life. Utilize established educational programs and social work interventions to address barriers and facilitate treatment objectives. Provide ongoing support and counseling to assist patients in adjusting to dialysis and achieving optimal psychosocial well-being. Quality Assurance: Deliver psychosocial interventions to address issues related to treatment adherence and quality of life. Participate in monthly quality review meetings, reporting on key indicators such as treatment adherence and patient satisfaction trends. Patient Education: Evaluate patients' understanding of kidney disease and tailor educational efforts to overcome barriers to treatment adherence. Collaborate with the healthcare team to provide comprehensive information on treatment options and facilitate the transplant referral process. Ensure patients and families are informed about psychosocial aspects of End Stage Renal Disease (ESRD) and available support services. Patient Admission and Continuity of Care: Review essential patient rights and responsibilities, addressing any immediate concerns. Understand the referral and admission processes to support patient scheduling and continuity of care. Insurance and Financial Guidance: Act as a liaison between patients and Insurance Coordinators to resolve insurance-related issues. Provide education on payment options for dialysis and assist patients in navigating financial assistance programs. Staff Collaboration: Assist in the recruitment process for new personnel as needed. Work with administrative staff to maintain updated resources for patient support. Provide training on psychosocial topics to enhance staff knowledge and contribute to team discussions on urgent patient matters. Adherence to Guidelines: Follow established caseload guidelines in accordance with state regulations.
