OBJECTIVE AND SCOPE: The student intern plays a vital role in supporting the mission, vision, core values, and customer service principles of Fresenius Medical Care. This position adheres to the FMCNA Compliance Program, ensuring compliance with all regulatory and policy requirements. The intern collaborates with the hemodialysis healthcare team to deliver safe and effective dialysis therapy for patients under the direct supervision of a licensed social worker, in alignment with FMCNA policies, procedures, and training, as well as state and federal regulations. The intern also contributes to maintaining a safe and hygienic working environment. Furthermore, the intern supports the FMCNA commitment to the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) initiatives, actively participating in activities that enhance patient satisfaction and improve processes to achieve FMC Quality Enhancement Goals (QEP). RESPONSIBILITIES AND DUTIES: This internship is a crucial component of Social Work education, promoting students' alignment with the profession's purposes, values, and ethics; fostering the integration of empirical knowledge with practical experience; and encouraging the development of professional competence as outlined by the CSWE Educational Policy and Accreditation Standards. A minimum number of required field education hours is established by the CSWE. Under the direct supervision of a Licensed Social Worker, the intern will be involved in:

Upholding the FMS culture through adherence to values and customer service standards. Ensuring exceptional customer service for all external and internal stakeholders. Building and maintaining effective relationships through timely and effective communication. Taking proactive measures to address, resolve, and follow up on customer service inquiries promptly.

PRINCIPAL RESPONSIBILITIES AND DUTIES RELATED TO PATIENT CARE:

As part of the interdisciplinary team, evaluates patients' psychosocial status, strengths, and needs that may influence rehabilitation and treatment outcomes as part of a comprehensive patient assessment. Engages in care planning alongside the patient and healthcare team to identify effective interventions that support rehabilitation, treatment goals, and enhance quality of life. Utilizes established patient education programs, social work theories, and quality of life measurement tools as part of assessment and care planning to address barriers and meet treatment objectives. Implements outcome-driven psycho-educational counseling methods aimed at improving treatment adherence, vocational/educational rehabilitation, and overall quality of life. Provides supportive and goal-oriented counseling to patients pursuing transplant options. Evaluates patient understanding of advance directives; assists in accessing advance directive information and facilitates discussions regarding advance directive wishes with the healthcare team and family/support persons as needed. Offers general information regarding Do Not Resuscitate Orders and Advance Directives. Provides information and assists patients with referrals to community resources (e.g., home health services, vocational rehabilitation) to facilitate optimal treatment outcomes. Delivers usual care and/or Social Work Intensive to address non-adherence and quality of life concerns for all patients. Maintains knowledge of and adheres to FMCNA Social Work Policy, including documentation standards.

PATIENT EDUCATION:

Assesses patient knowledge of kidney disease to identify barriers affecting treatment adherence. Collaborates with patients, families, and the healthcare team to provide education tailored to individual learning styles and needs. Alongside other interdisciplinary team members, delivers appropriate information about all treatment modalities. Facilitates the transplant referral process and collaborates with the team as delegated by the Clinical Manager. Provides ongoing education to patients and families regarding psychosocial issues related to End Stage Renal Disease (ESRD) and available support services. Reviews patient rights and responsibilities, grievance procedures, and facility policies with patients and/or their representatives to ensure understanding of rights and expectations. Collaborates with the team on relevant Quality Assurance and Improvement activities.

INSURANCE AND FINANCIAL ASSISTANCE:

Keeps current on local vocational/educational rehabilitation programs and assists patients with referrals to enable them to maintain or gain employment or pursue education. Understands FMCNA financial policies and procedures related to patient services. Collaborates with Financial Coordinators to educate patients about available programs for uncovered services, billing concerns, and financial responsibilities.

STAFF RELATED:

Works with administrative support staff to maintain updated patient resource lists (including transportation resources). Develops and delivers training to staff on psychosocial topics as needed. Provides training on Ultra Care Programs as required. Performs additional duties as assigned.

PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and characteristics of the work environment described here are representative of those encountered while performing the essential functions of this position. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. Daily responsibilities include desk work and personal computer usage, along with interaction with patients, facility staff, and physicians. The role requires travel between assigned facilities and various community locations. Attendance at regional, Business Unit, and Corporate meetings may be necessary. The work environment is typical of a healthcare facility, featuring air temperature control and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur. EDUCATION: High School diploma or equivalent. EXPERIENCE AND REQUIRED SKILLS: Preferred experience in delivering outstanding customer service to all external and internal customers. A strong desire to provide superior care to patients and align with Fresenius' mission and values is essential. RELATIONSHIPS: The intern cannot work under the supervision of a family member and cannot care for a patient who is a family member. If a family member is a patient at the facility, the intern will be scheduled to work on days when the patient is not present or assigned to another patient. SUPERVISION: The Clinical Manager assigns and closely monitors all Social Work Intern assignments with direct supervision from a Social Worker Intern based on the agreed educational plan. OTHER: Performs additional duties as assigned.
