Key Responsibilities:
- Conduct comprehensive assessments of patients' psychosocial conditions, identifying strengths and needs that may influence rehabilitation and treatment outcomes.
- Collaborate with patients and the healthcare team to develop effective interventions aimed at achieving rehabilitation and treatment objectives while enhancing quality of life.
- Implement patient education initiatives, utilizing established social work theories and methods to address barriers and facilitate treatment adherence.
- Monitor and provide interventions to assist patients in adjusting to dialysis, ensuring optimal psychosocial well-being.
- Evaluate patients' understanding of kidney disease and identify barriers to treatment compliance.
- Offer supportive counseling services within the scope of your clinical training and state licensure.
- Assist patients and the healthcare team in accessing community resources to optimize treatment outcomes.
- Stay informed about local vocational and educational rehabilitation programs, guiding patients in their pursuit of employment or education.
- Facilitate discussions regarding advance directives, ensuring patients and families are informed about their care options.
- Participate in discussions surrounding patients' DNR status, ensuring informed decision-making.
- Report on quality indicators related to treatment adherence and patient quality of life.
- Tailor educational efforts to meet patients' unique learning styles and communication needs.
- Provide counseling and education to patients pursuing transplant options.
- Collaborate with the interdisciplinary team on transplant waitlist management.
- Educate patients and families about psychosocial issues related to end-stage renal disease (ESRD) and available support services.
- Review patient rights and responsibilities, ensuring understanding of facility policies.
- Deliver training to clinic staff on relevant psychosocial topics as necessary.
Required Qualifications:
- Master’s degree in Social Work.
- Valid state license as required.
Experience and Skills:
- 0 – 2 years of relevant experience.
