Position Overview:

As a Clinical Social Worker at Fresenius Medical Care, you will deliver essential psychosocial support to patients in our dialysis facilities. Collaborating closely with the healthcare team, your role will focus on enhancing the rehabilitation process and fostering a better quality of life for patients facing chronic kidney disease. You will also assist clinic personnel in comprehending the emotional, psychological, and behavioral challenges that accompany this condition for both patients and their families.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' psychosocial conditions, identifying strengths and needs that may influence treatment outcomes.
  • Collaborate with patients and the healthcare team to devise effective interventions aimed at achieving rehabilitation and treatment objectives.
  • Implement educational programs and established social work methodologies to address barriers and facilitate patient treatment goals.
  • Provide ongoing support and interventions to help patients adapt to dialysis and enhance their psychosocial well-being.
  • Evaluate patients' understanding of kidney disease and identify potential barriers to treatment adherence.
  • Offer supportive counseling services within the scope of your clinical training and state licensure.
  • Assist patients in accessing community resources, such as home health services and vocational rehabilitation, to optimize treatment outcomes.
  • Stay informed about local vocational and educational rehabilitation programs to help patients maintain or regain employment.
  • Facilitate discussions regarding advance directives and ensure patients and families understand their rights and options.
  • Participate in discussions about patients' DNR status to ensure informed decision-making regarding care.
  • Monitor quality indicators related to treatment adherence and report on trends affecting patient care.
  • Educate patients and families about psychosocial issues related to end-stage renal disease (ESRD) and available support services.
  • Provide training to clinic staff on psychosocial topics as necessary.

Qualifications:

  • Master’s degree in Social Work.
  • Valid state licensure as required.

Experience and Skills:

  • 0 – 2 years of relevant experience in social work.
