OBJECTIVE AND SCOPE: The student intern plays a vital role in supporting the mission, vision, core values, and customer service principles of Fresenius Medical Care. This position adheres to the FMCNA Compliance Program, ensuring all regulatory and policy requirements are met. The intern collaborates with the hemodialysis healthcare team to deliver safe and effective dialysis therapy for patients, under the direct supervision of a licensed social worker, in accordance with FMCNA policies, procedures, and training, while complying with state and federal regulations. The intern is also responsible for maintaining a safe and clean working environment. This role supports the FMCNA commitment to the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) initiatives, actively participating in activities that enhance patient satisfaction and contribute to achieving FMC Quality Enhancement Goals (QEP). RESPONSIBILITIES AND DUTIES: This internship is a crucial component of Social Work education, promoting students' alignment with the profession's purposes, values, and ethics; fostering the integration of empirical knowledge with practical experience; and encouraging the development of professional competence as outlined by the CSWE Educational Policy and Accreditation Standards. A minimum number of required field education hours are established by the CSWE. The social work intern, under the direct guidance of a Licensed Social Worker, will engage in the following:

Cultivate and maintain effective relationships through timely and effective communication. Proactively address, resolve, and follow up on customer service issues in a timely manner.PRINCIPAL RESPONSIBILITIES AND PATIENT CARE DUTIES:
As part of the interdisciplinary team, assess patients' psychosocial status, strengths, and needs that may impact rehabilitation and treatment outcomes as part of the comprehensive patient assessment. Collaborate with patients and the healthcare team in care planning to identify effective interventions that support rehabilitation and treatment goals, enhancing quality of life. Utilize established patient education programs, social work theories, and quality of life measurement tools in assessment and care planning to address barriers and meet treatment objectives. Implement psycho-educational counseling methods aimed at improving treatment adherence, vocational/educational rehabilitation, and overall quality of life. Provide supportive and goal-directed counseling to patients pursuing transplant options. Assess patient understanding of advance directives and facilitate discussions regarding advance directive wishes with the healthcare team and family/support persons. Offer general information about Do Not Resuscitate Orders and Advance Directives. Assist patients with referrals to community resources (e.g., home health services, vocational rehabilitation) to optimize treatment outcomes. Deliver usual care and/or Social Work Intensive services to address non-adherence and quality of life concerns for all patients. Adhere to FMCNA Social Work Policy, including documentation requirements.PATIENT EDUCATION:
Evaluate patient knowledge of kidney disease to identify barriers affecting treatment adherence. Collaborate with patients, families, and the healthcare team to provide tailored education based on individual learning styles and needs. Provide comprehensive information about all treatment modalities and facilitate the transplant referral process in collaboration with the interdisciplinary team, as delegated by the Clinical Manager. Offer ongoing education to patients and families regarding psychosocial issues related to End Stage Renal Disease (ESRD) and available support services. Review patient rights and responsibilities, grievance procedures, and facility policies with patients and/or their representatives to ensure understanding of rights and expectations. Collaborate with the team on relevant Quality Assurance and Improvement activities.INSURANCE AND FINANCIAL ASSISTANCE:
Stay informed about local vocational/educational rehabilitation programs and assist patients with referrals to enable them to maintain or pursue employment or education. Understand FMCNA financial policies and procedures related to patient services. Collaborate with Financial Coordinators to educate patients about available programs for uncovered services, billing concerns, and financial responsibilities.STAFF RELATED:
Work with administrative support staff to maintain updated patient resource lists, including transportation resources. Develop and provide training to staff on psychosocial topics as needed. Train staff in Ultra Care Programs as required. Perform additional duties as assigned.PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and characteristics of the work environment described here are representative of those encountered while performing the essential functions of this position. Daily responsibilities include desk work, personal computer use, and interaction with patients, facility staff, and physicians. The role requires travel between assigned facilities and various community locations. Attendance at regional, Business Unit, and Corporate meetings may be necessary. The work environment is typical of a healthcare facility, featuring air temperature control and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur. EDUCATION: High School diploma or equivalent. EXPERIENCE AND REQUIRED SKILLS: Exceptional customer service skills for all external and internal customers. A strong desire to provide superior care to patients while embracing Fresenius' mission and values. RELATIONSHIPS: The Intern cannot work under the supervision of a family member and cannot care for a patient who is a family member. If a family member is a patient at the facility, the Intern will be scheduled to work on days the patient is not present or assigned to another patient. SUPERVISION: The Clinical Manager assigns and closely monitors all Social Work Intern assignments with direct supervision from a Social Worker based on the agreed education plan. OTHER: Additional duties may be assigned as necessary.
