Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Therapies team at Fresenius Medical Care. As a Home Therapies Registered Nurse, you will play a critical role in managing patient care in our independent home therapy program. Your primary responsibility will be to provide education and support to patients and their caregivers who have chosen to dialyze in the comfort of their own homes.
Key Responsibilities:- Conduct initial visits to patients' homes to assess their readiness for home therapy and provide training and support as needed.
- Develop and implement individualized care plans to ensure patients receive the best possible treatment and achieve optimal health outcomes.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide ongoing education and support to patients and their caregivers to ensure they are confident and competent in performing dialysis treatments at home.
- Monitor patients' progress and adjust treatment plans as needed to ensure optimal health outcomes.
- Participate in quality improvement initiatives to enhance patient care and program operations.
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet the practice requirements in the state in which he or she is employed.

- Experience as a Registered Nurse (preferred but not required).
- Chronic/home hemodialysis experience (preferred but not required).
