POSITION SUMMARY:
The Home Therapies Registered Nurse (RN) CAP 1 is an entry-level role within the Clinical Advancement Program (CAP). This position is responsible for delivering and coordinating competent clinical care, which includes assessment, planning, intervention, and evaluation for a designated group of patients. The RN assesses and manages patient responses to home dialysis training and treatment protocols, ensuring effective communication of patient-related concerns to the physician as necessary. As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, this role engages in decision-making, educational initiatives, leadership responsibilities, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.
KEY RESPONSIBILITIES:
All responsibilities must be executed in alignment with Fresenius Kidney Care policies, nursing standards, and applicable regulations.
- Execute essential functions under the supervision of the Supervisor, with support from the Educator, Preceptor, or in collaboration with fellow Registered Nurses.
- Conduct ongoing, systematic collection and analysis of dialysis data for assigned patients, documenting findings in the medical record, and adjusting treatment plans as necessary while notifying the Supervisor or physician.
- Evaluate and document patient and family learning needs to provide both initial and continuous education.
- Collaborate with patients, home care partners, and ancillary staff to ensure the safe and effective delivery of dialysis therapy.
- Administer prescribed medications in accordance with established protocols and document the medical rationale and outcomes.
- Initiate or assist in emergency response measures as required.
- Serve as a resource for the healthcare team and participate in training and orientation for new staff as assigned.
- Ensure proper laboratory collection, processing, and shipping procedures are adhered to, rescheduling any missed or insufficient collections.
- Identify expected outcomes and update nursing assessments and care plans in collaboration with the Interdisciplinary Team.
- Facilitate patient awareness regarding transplant and treatment modality options.
- Assist in the evaluation and education of Home Dialysis candidates and their partners.
- Assess and identify barriers in the home environment and readiness of candidates and their families for dialysis treatments.
- Train Home Dialysis patients and/or Home Partners on the safe operation and maintenance of Home Dialysis equipment through a structured training program.
- Complete CAP requirements for advancement.
- Perform additional duties as assigned by the Supervisor.
WORK ENVIRONMENT AND PHYSICAL DEMANDS:

- This role involves direct patient care, which may require heavy lifting and assistance with patient mobility.
- Frequent prolonged periods of standing are necessary, along with the ability to bend and lift.
- Occasionally, the employee may need to move equipment weighing up to 200 lbs. and lift solutions weighing up to 30 lbs.
- The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels.
- Exposure to infectious and contagious materials may occur.
- Coverage rotation with other licensed home therapies staff is required to ensure adequate service delivery.
- Participation in on-call rotation and varied shifts may be necessary.
- Travel to training sites or patient homes may be required.
- Daily responsibilities include desk work, computer tasks, and interaction with patients and healthcare staff.
SUPERVISORY RESPONSIBILITIES:
Oversight of LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants may be required as a Team Leader or designated Nurse in charge, contingent upon meeting all necessary qualifications.
- Successful completion of all required education and training for new employees.
- A minimum of 12 months of RN experience is required.
- Completion of 3 months of experience in home peritoneal dialysis and/or hemodialysis is necessary.
EDUCATION AND LICENSURE:
- Graduate of an accredited Nursing School.
- Current state licensure is required.
- Current or successful completion of CPR BLS Certification is mandatory.
- Must meet practice requirements in the state of employment.
EXPERIENCE AND SKILLS:
- Entry-level for RNs with less than 1 year of Nephrology Nursing experience in the past two years.
- Preferred minimum of 1-year experience as a Registered Nurse.
- Home dialysis therapy experience is preferred.
