Home Dialysis Registered Nurse

POSITION SUMMARY:
The Registered Nurse specializing in Home Therapies plays a crucial role in the Clinical Advancement Program (CAP). This position is dedicated to delivering high-quality clinical care to patients by assessing their needs, planning appropriate interventions, and evaluating outcomes. The nurse is responsible for monitoring patients' responses to home dialysis training and treatment, adhering to established protocols, and communicating any concerns to the physician as needed. As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, this role encompasses decision-making, educational responsibilities, leadership tasks, and quality improvement initiatives aimed at enhancing patient care and operational efficiency.
KEY RESPONSIBILITIES:
- Conducts essential nursing functions under supervision
- Collects and interprets dialysis data, modifies treatment plans, and informs the Supervisor or physician as necessary
- Evaluates and documents patient and family educational needs, providing necessary training
- Oversees and ensures the safe and effective administration of dialysis therapy
- Dispenses medications as prescribed by the physician
- Initiates or assists in emergency procedures when required
- Acts as a resource for staff, participates in training sessions, and ensures adherence to laboratory protocols
- Identifies expected patient outcomes, updates nursing assessments, and revises care plans accordingly
- Informs patients about transplant and treatment alternatives
- Assists in the selection of candidates for Home Dialysis and provides necessary training
- Recognizes barriers within the home environment and educates patients and caregivers on equipment usage
- Completes CAP requirements as outlined
- Performs additional assigned duties as required
WORKING CONDITIONS AND PHYSICAL DEMANDS:
- Involves direct patient care requiring significant lifting and movement of equipment
- Requires extended periods of standing
- May involve handling heavy equipment and exposure to infectious diseases
- Participation in on-call rotation and potential travel may be necessary
SUPERVISORY RESPONSIBILITIES:
- Oversees the work of LPNs/LVNs, RNs, Patient Care Technicians, and Assistants
- Must fulfill specific experience criteria
EDUCATIONAL REQUIREMENTS AND LICENSURE:
- Graduate from an accredited School of Nursing
- Current state nursing license
- CPR BLS Certification
- Must comply with state practice regulations
EXPERIENCE AND REQUIRED SKILLS:
- Entry-level position for RNs with limited experience in Nephrology Nursing
- Preferred minimum of 1-year experience as a Registered Nurse
- Preferred experience in home dialysis therapy
