POSITION SUMMARY:
The Home Therapies Registered Nurse (RN) CAP 1 is an entry-level role within the Clinical Advancement Program (CAP). This position is responsible for delivering and coordinating high-quality care, including assessment, planning, intervention, and evaluation for a designated group of patients. The RN will evaluate and manage patients' responses to home dialysis training and treatment by adhering to established protocols and will communicate any patient-related concerns to the physician as necessary. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role engages in decision-making, education, leadership, and quality improvement initiatives aimed at enhancing patient care outcomes and operational efficiency.
KEY RESPONSIBILITIES:
The following duties and responsibilities are to be performed in alignment with Fresenius Kidney Care policies, nursing standards, and applicable regulations:
- Execute essential functions under the supervision of the Supervisor, with support from the Educator, Preceptor, or in collaboration with another Registered Nurse.
- Conduct ongoing and systematic collection and analysis of dialysis data for assigned patients, documenting findings in the medical record, adjusting treatment plans as necessary, and notifying the Supervisor or physician when required.
- Assess and document the educational needs of patients and families to provide initial and ongoing education.
- Collaborate with patients, home care partners, and ancillary staff to ensure safe and effective delivery of dialysis therapy.
- Administer medications as prescribed and document the medical justification and effectiveness.
- Initiate or assist with emergency response protocols as needed.
- Serve as a resource for the healthcare team and participate in training and orientation for new staff.
- Ensure proper laboratory collection and processing procedures are followed, rescheduling any missed or insufficient collections.
- Identify expected outcomes and update nursing assessments and care plans in collaboration with the Interdisciplinary Team.
- Facilitate patient awareness regarding transplant and treatment modality options.
- Assist in identifying and educating potential Home Dialysis candidates and their partners.
- Evaluate the home environment and readiness of candidates and partners for home dialysis treatments.
- Provide training to Home Dialysis patients and/or Home Partners on the safe operation and maintenance of dialysis equipment through a structured training program.
- Complete CAP requirements for advancement.
- Perform additional duties as assigned by the Supervisor.
WORK ENVIRONMENT AND PHYSICAL DEMANDS:

- This role involves direct patient care, which may require heavy lifting and moving of equipment.
- Frequent and prolonged periods of standing are necessary, along with the ability to bend over.
- Occasionally, the employee may need to move equipment weighing up to 200 lbs. and lift solutions weighing up to 30 lbs.
- The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels.
- Exposure to infectious and contagious diseases/materials may occur.
- Participation in on-call rotation and coverage with other licensed staff is required.
- Travel to various training sites or patient homes may be necessary.
- Daily tasks include desk work, computer use, and interaction with patients and healthcare staff.
SUPERVISORY RESPONSIBILITIES:
This position may involve oversight of LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge, contingent upon meeting specific criteria.
EDUCATION AND LICENSURE:
- Graduate of an accredited School of Nursing.
- Current state licensure as a Registered Nurse.
- Current or successful completion of CPR BLS Certification.
- Must meet practice requirements in the state of employment.
EXPERIENCE AND REQUIRED SKILLS:
- Entry-level for RNs with less than 1 year of Nephrology Nursing experience within the last two years.
- Preferred minimum of 1-year experience as a Registered Nurse.
- Home dialysis therapy experience is preferred.
