POSITION SUMMARY:

The Registered Dietitian is responsible for delivering nutritional care to patients in both in-center and home settings, aiming to enhance the nutritional well-being of individuals and optimize health outcomes. This role is integral to the multidisciplinary healthcare team, contributing to the organization's commitment to quality improvement initiatives and patient-centered care.

KEY RESPONSIBILITIES:

  • Evaluates patients' understanding of dietary needs related to kidney health and provides tailored education to meet their learning preferences.
  • Offers comprehensive nutritional guidance to patients, families, and caregivers regarding dietary status, adequacy measures, fluid management, and relevant laboratory results.
  • Conducts thorough nutritional assessments and collaborates in comprehensive evaluations with the healthcare team.
  • Engages with the interdisciplinary team to formulate individualized care plans that address patients' specific requirements, adhering to federal and state regulations.
  • Develops diet prescriptions in accordance with established standards.
  • Maintains detailed progress notes for all chronic dialysis patients, ensuring compliance with organizational policies and regulatory requirements.
  • Provides continuous nutrition assessments and counseling aimed at helping patients achieve and maintain optimal nutritional health.
  • Identifies patients at risk for malnutrition and collaborates with the healthcare team to implement effective interventions.
  • Monitors patient adherence to nutritional therapy and adjusts treatment plans as necessary to support effective health outcomes.
  • Works alongside the healthcare team to provide recommendations for managing mineral metabolism, renal bone disease, anemia, and other related health issues.
  • Complies with organizational protocols and documentation standards.
  • Participates in quality assurance initiatives and contributes to the interdisciplinary quality improvement program.
  • Collaborates on educational initiatives for patients in the pre-ESRD phase as appropriate.
  • Reviews laboratory results related to nutrition and provides actionable recommendations to the healthcare team.
  • Engages in relevant staff meetings and care plan discussions to ensure comprehensive patient care.
  • Consults with corporate dietitians on complex nutritional concerns as needed.
  • Continuously enhances professional knowledge and skills to maintain a high standard of practice.
  • Ensures compliance with dietetic registration and continuing education requirements as mandated by relevant authorities.
  • Collaborates with team members to maintain a safe and clean working environment.
  • Performs additional nutrition management tasks as assigned.

WORK ENVIRONMENT:

  • The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels.
  • May involve exposure to infectious diseases and requires travel between various healthcare locations.

EDUCATION AND CREDENTIALS:

  • Must be a Registered Dietitian as recognized by the Commission on Dietetic Registration.
  • Board Certification in Renal Nutrition is preferred.
  • Current state licensure is required where applicable.

EXPERIENCE AND SKILLS:

  • A minimum of one year of professional experience in clinical nutrition as a Registered Dietitian is required.
  • Previous experience in renal nutrition is preferred.
  • Strong leadership, communication, and organizational skills are essential for success in this role.
  • Ability to work independently and manage responsibilities with minimal supervision.
  • Basic computer proficiency is required.
