POSITION SUMMARY:
The Registered Dietitian plays a crucial role in delivering nutritional care to patients within both in-center and home settings. This position is essential for enhancing the nutritional well-being of patients and fostering improved health outcomes. As part of a collaborative healthcare team, the Registered Dietitian contributes to the organization's commitment to quality improvement initiatives and patient-centered care.
KEY RESPONSIBILITIES:
- Evaluates patients' understanding of dietary needs related to kidney health and provides tailored education to meet individual learning requirements.
- Delivers comprehensive nutritional guidance to patients, families, and caregivers, focusing on nutritional status, dietary adequacy, fluid management, and relevant laboratory results.
- Conducts thorough nutritional assessments and collaborates in comprehensive interdisciplinary evaluations.
- Engages with the healthcare team to formulate a personalized care plan that addresses the unique needs of each patient, adhering to federal and state regulations.
- Determines dietary prescriptions based on established clinical practices.
- Maintains accurate documentation of patient progress in accordance with organizational policies and regulatory standards.
- Provides ongoing nutritional evaluations and counseling to support patients in achieving and maintaining optimal nutritional health.
- Identifies patients at risk for malnutrition and collaborates with the healthcare team to implement appropriate interventions.
- Monitors patients' adherence to nutritional therapies and adjusts plans as necessary to ensure effective dietary management.
- Works with the interdisciplinary team to assess and manage mineral metabolism, renal bone disease, anemia, and other nutrition-related health issues.
- Participates in quality assessment and improvement initiatives related to nutrition.
- Coordinates with team members to provide education for patients at risk of end-stage renal disease.
- Reviews laboratory results and communicates findings to patients and the healthcare team, recommending necessary adjustments to treatment plans.
- Attends relevant meetings and collaborates with corporate dietitians for additional support and guidance.
- Continuously enhances professional knowledge and skills to ensure high-quality nutrition care.
- Maintains dietetic registration and fulfills continuing education requirements as mandated by professional organizations and state regulations.
- Ensures a safe and clean working environment in collaboration with the healthcare team.
- Performs additional nutrition management tasks as required.
WORK ENVIRONMENT:
The role is performed in a healthcare setting characterized by controlled air temperature and moderate noise levels. Employees may encounter infectious diseases and are required to travel between various facilities and community locations.
EDUCATION AND CREDENTIALS:
- Must be a Registered Dietitian as recognized by the Commission on Dietetic Registration.
- Board Certification in Renal Nutrition is preferred.
- Current state licensure is required where applicable.
EXPERIENCE AND SKILLS:
- A minimum of one year of professional experience in clinical nutrition as a Registered Dietitian is required.
- Previous experience in renal nutrition is preferred.
- Strong leadership skills, effective communication abilities, and a commitment to patient-centered care are essential.
- Ability to work independently with minimal supervision.
- Proficiency in basic computer skills is necessary.
