PURPOSE AND SCOPE:

The Clinical Nutrition Specialist in training, possessing less than one year of hands-on clinical experience, will collaborate closely with a certified dietitian. Under the direct guidance of an experienced renal dietitian, this role aims to deliver nutritional support for both in-center and home patients, gaining the essential experience mandated by CMS until the requisite clinical experience is fulfilled to qualify as a registered dietitian.

This position actively engages in initiatives aimed at process enhancement to boost the probability that patients will reach their individualized health objectives as established by their healthcare provider. The Dietitian in training will not have the qualifications to conduct patient evaluations, formulate care plans, or oversee QAPI program reviews. This role operates under the direct supervision of a company Registered Dietitian with a minimum of three years of renal dietetic experience. The position supports the FMCNA's commitment to the Quality Enhancement Program (QEP).

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Uphold and promote the company culture through adherence to values and customer service standards. Ensure exceptional customer service for all external and internal stakeholders. Foster and maintain effective relationships through timely and clear communication. Take proactive measures to address, resolve, and follow up on customer service inquiries promptly. Calculate dietary prescriptions in accordance with established standards of practice. Assess patients' understanding of dietary needs and kidney health. Complete Nutrition History documentation with patients for use by CMS-qualified dietitians in assessments. Provide tailored education to patients, families, and caregivers regarding nutritional status, adequacy measures, fluid management, and relevant lab results, considering the patient's preferred learning style. Analyze nutrition-related lab results: Counsel patients and/or caregivers and devise appropriate strategies to help patients achieve and maintain optimal nutritional health. Communicate lab findings and suggested treatment modifications to the interdisciplinary team as necessary. Review each patient's albumin levels and body weight monthly. Identify patients at risk for malnutrition and collaborate with the interdisciplinary team to determine suitable interventions and resources. Involve patients and their families in goal-setting for improvement. When suitable, identify candidates for nutritional supplement referrals to physicians based on disease management agreements. Assist in managing patients' fluid and volume levels. Provide education on fluid and sodium intake as needed. Collaborate with the interdisciplinary team on recommendations for managing mineral metabolism, renal bone disease, anemia, nutritional status, psychosocial factors, vascular access, treatment modalities, rehabilitation, and sodium/volume control. Participate in Quality Assessment and Performance Improvement and Plan of Care meetings alongside a CMS-qualified dietitian. Attend relevant staff meetings as required. Work with the interdisciplinary team to ensure all workspaces are safe and well-maintained. Collaborate with the interdisciplinary team on pharmacy-related responsibilities, including setup, enrollment, and management. Engage in community outreach activities as needed. Educate patients on emergency preparedness concerning food and nutrition. Conduct staff training sessions as necessary or requested. Adhere to FMCNA protocols and medical record policies. Continuously enhance knowledge and skills for a competent and innovative practice. Maintain dietetic registration and fulfill continuing education requirements as stipulated by the American Dietetic Association and state licensure regulations where applicable. Perform additional nutrition service duties as assigned. Escalate issues to supervisors for resolution when necessary. Assist with various projects as directed by the immediate supervisor. Execute other related responsibilities as assigned.

The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels. Employees may be exposed to infectious and contagious diseases. Daily tasks include desk work and interaction with patients and facility staff, and the position may require travel between assigned facilities and various locations within the community. Attendance at training clinics and regional, Business Unit, and Corporate meetings may also be necessary.

EDUCATION AND REQUIRED CREDENTIALS :

Bachelor's degree in Nutrition or a closely related field; Master's degree preferred. Registered Dietitian credential from the Commission on Dietetic Registration. Current state licensure as applicable.

Less than one year of clinical experience.
