POSITION SUMMARY:
The Clinical Nutrition Specialist is responsible for delivering nutritional care under the guidance of a seasoned dietitian. This role is designed for individuals in training with less than one year of clinical experience, focusing on providing nutritional support to both in-center and home patients. The primary goal is to acquire the necessary experience as mandated by CMS to transition into a fully qualified dietitian role.
This position actively engages in initiatives aimed at improving patient outcomes, ensuring that each patient meets their personalized health objectives as outlined by their healthcare provider. The Clinical Nutrition Specialist will not conduct patient assessments or develop care plans independently but will work closely with a qualified dietitian who has a minimum of three years of renal dietetic experience. This role supports the organization's commitment to the Quality Enhancement Program (QEP).
KEY RESPONSIBILITIES:
Uphold the company culture through adherence to core values and exceptional customer service standards. Ensure high-quality service delivery to all internal and external stakeholders. Foster and maintain strong relationships through effective communication. Proactively address and resolve customer service inquiries in a timely manner. Calculate dietary prescriptions in accordance with established standards. Assess patients' understanding of dietary needs related to kidney health. Complete Nutrition History documentation for use by CMS-qualified dietitians in assessments. Provide tailored educational resources to patients and their families regarding nutritional health, fluid management, and relevant laboratory results, considering individual learning preferences. Analyze nutrition-related lab results and collaborate with patients and caregivers to develop actionable strategies for improving nutritional status. Communicate findings and recommended adjustments to the interdisciplinary team as necessary. Conduct monthly reviews of patients' albumin levels and body weight, identifying those at risk for malnutrition and collaborating with the healthcare team to implement appropriate interventions. Identify potential candidates for nutritional supplementation referrals when indicated. Assist in managing patients' fluid and volume needs. Provide education on fluid and sodium intake as needed. Work alongside the interdisciplinary team to manage mineral metabolism, renal bone health, anemia, and other nutritional concerns. Participate in Quality Assessment and Performance Improvement meetings with a CMS-qualified dietitian. Attend relevant staff meetings as required. Ensure all workspaces are safe and well-maintained in collaboration with the healthcare team. Engage in pharmacy-related responsibilities as part of the interdisciplinary team. Participate in community outreach events as needed. Educate patients on emergency preparedness related to nutrition. Conduct training sessions for staff as requested. Adhere to company protocols and medical record policies. Continuously enhance professional knowledge and skills to maintain a high standard of practice. Maintain dietetic registration and fulfill continuing education requirements as per the American Dietetic Association and state regulations. Perform additional nutrition service duties as assigned. Escalate issues to supervisors when necessary. Assist with various projects as directed by supervisors. Carry out other related responsibilities as needed.WORK ENVIRONMENT:
The work environment is typical of a healthcare facility, featuring climate control and moderate noise levels. Employees may encounter infectious diseases and will engage in desk work, computer use, and patient interactions. Travel between facilities and to training events may be required.EDUCATION AND CREDENTIALS:
Bachelor’s degree in Nutrition or a related field; Master’s degree preferred. Registered Dietitian credential from the Commission on Dietetic Registration. Current state licensure as applicable.EXPERIENCE AND SKILLS:
