PURPOSE AND SCOPE:
The primary function of this role is to deliver nutritional services tailored for both in-center and home patients, aiming to enhance the nutritional health of individuals and promote positive clinical outcomes. This position is integral to the interdisciplinary healthcare team, contributing to the FMCNA's dedication to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) initiatives. The role involves active participation in process improvement efforts to ensure patients meet their personalized health objectives as outlined by their healthcare providers.
KEY RESPONSIBILITIES:
Evaluates patients' understanding of dietary needs related to kidney health and provides education tailored to their learning preferences, emphasizing the connection between nutrition and kidney disease. Delivers detailed, patient-specific education to patients, families, and caregivers concerning nutritional status, adequacy measures, fluid management, and relevant laboratory results associated with their condition. Conducts comprehensive nutritional assessments and collaborates in interdisciplinary evaluations. Works with the healthcare team to formulate a personalized, comprehensive care plan that addresses the unique needs of each patient, adhering to established federal and state guidelines. Determines dietary prescriptions in accordance with best practices. Documents progress notes for all chronic dialysis patients, ensuring compliance with FMCNA protocols and maintaining accurate medical records as per regulatory standards. Provides continuous nutrition assessments and outcome-focused counseling to support patients in achieving and maintaining optimal nutritional health. Identifies patients at risk of malnutrition and collaborates with the healthcare team to devise suitable interventions and resources. Reviews patients' albumin levels and body weight monthly, implementing care plans with the involvement of patients and their families. Monitors patients' adherence to nutrition therapy, making necessary adjustments to maintain effective nutritional status, and documents any barriers to achieving health goals. Collaborates with the healthcare team on Urea Kinetic Modeling to fulfill the physician's Kt/V prescription. Engages with the interdisciplinary team on managing mineral metabolism, renal bone disease, anemia, nutritional status, psychosocial factors, vascular access, treatment modalities, rehabilitation, and sodium/volume control. Follows FMCNA protocols and medical record policies diligently. Participates in reporting nutrition QAI outcomes and engages in the interdisciplinary QAI program. Coordinates with team members to provide Pre-ESRD education as needed. Collaborates with Case Managers to align educational plans and identifies candidates for nutritional supplement referrals as per disease management agreements. Reviews and interprets nutrition-related laboratory results: Advises patients and/or caregivers and formulates appropriate action plans. Suggests treatment modifications to the interdisciplinary team as necessary. Communicates relevant lab results and treatment recommendations to healthcare providers in various settings. Participates in essential staff meetings, care plan discussions, and other relevant gatherings. Consults with the Corporate Dietitian regarding nutrition-related concerns as required. Continuously enhances knowledge and skills to maintain a competent and innovative practice. Upholds dietetic registration and fulfills continuing education requirements as mandated by the American Dietetic Association and applicable state regulations. Collaborates with the interdisciplinary team to ensure a safe and clean working environment. Undertakes additional nutrition management responsibilities as assigned. Registered Dietitian as recognized by the Commission on Dietetic Registration. Board Certified Specialist in Renal Nutrition is preferred. Current state licensure as applicable.EXPERIENCE AND SKILLS :
At least 1 year of professional experience in clinical nutrition as a Registered Dietitian. Previous experience in renal nutrition is advantageous. Demonstrated leadership skills and competencies relevant to the position, including teaching ability, strong organizational skills, excellent communication, customer service, relationship development, results orientation, team collaboration, and decision-making capabilities. Ability to perform responsibilities with minimal supervision.
