POSITION SUMMARY:
The Home Therapies Registered Nurse (RN) is an essential role within the Clinical Advancement Program (CAP), focusing on the delivery and management of competent clinical care for patients undergoing home dialysis. This position is responsible for evaluating, planning, implementing, and assessing care for a designated patient group. The RN will oversee patient responses to home dialysis training and therapy, adhering to established protocols, and will communicate any patient-related concerns to the physician as necessary. As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, this role involves active participation in decision-making, educational initiatives, leadership responsibilities, and quality improvement efforts aimed at enhancing patient care outcomes.
KEY RESPONSIBILITIES:
The following duties are expected to be performed in alignment with Fresenius Kidney Care policies, nursing standards, and regulatory requirements:
- Execute essential nursing functions under the supervision of the Team Leader, with support from the Educator or in collaboration with other RNs.
- Conduct systematic assessments and documentation of dialysis data for assigned patients, adjusting treatment plans as necessary, and informing the Supervisor or physician of any changes.
- Evaluate and document the educational needs of patients and their families to provide comprehensive and ongoing training.
- Collaborate with patients, caregivers, and healthcare staff to ensure the safe and effective delivery of dialysis treatments.
- Administer prescribed medications and document their effectiveness in accordance with established guidelines.
- Participate in emergency response protocols as required.
- Act as a resource for the healthcare team and assist in the training and orientation of new staff members.
- Ensure proper laboratory procedures are followed and manage any missed or insufficient collections.
- Identify and document expected patient outcomes, updating care plans in collaboration with the Interdisciplinary Team.
- Educate patients about transplant options and treatment modalities.
- Assist in the selection and training of Home Dialysis candidates and their support partners.
- Assess the home environment and readiness of candidates and their families for home dialysis treatments.
- Provide formal training on the operation and maintenance of home dialysis equipment and supplies.
- Complete all CAP requirements for professional advancement.
- Perform additional duties as assigned by the Supervisor.
WORK ENVIRONMENT AND PHYSICAL DEMANDS:
The physical and environmental characteristics of this role include:
- Direct patient care involving heavy lifting and movement of equipment.
- Frequent standing and bending, with occasional lifting of equipment weighing up to 200 lbs.
- Exposure to a healthcare facility environment with controlled air temperature and moderate noise levels.
- Potential exposure to infectious materials.
- Participation in on-call rotations and travel to various locations as needed.
- Daily responsibilities include desk work, computer usage, and patient interaction.
SUPERVISORY RESPONSIBILITIES:
The RN may oversee LPNs, LVNs, Patient Care Technicians, and Home Therapy Care Team Assistants, contingent upon meeting the necessary qualifications and experience.
EDUCATION AND LICENSURE:
- Graduate of an accredited nursing program.
- Valid state nursing license.
- Current CPR BLS Certification.
EXPERIENCE AND SKILLS:
- Entry-level position for RNs with less than one year of nephrology nursing experience.
- Preferred minimum of one year of experience as a Registered Nurse.
- Experience in home dialysis therapy is preferred.
