Healthcare Professional: Ambulatory Care Specialist

About the Role
We are seeking a skilled and experienced healthcare professional to join our team at PubliCare as an Ambulatory Care Specialist. In this role, you will be responsible for providing patient education and support in the community, working closely with healthcare providers and other stakeholders to ensure seamless care transitions.
- You will be responsible for conducting patient assessments, developing care plans, and providing education and support to patients and their families in the community.
- You will work closely with healthcare providers to ensure that patients receive the necessary care and support in the community, and will be responsible for coordinating care transitions between healthcare settings.
- You will be responsible for maintaining accurate records and reporting on patient outcomes, and will work closely with the healthcare team to identify areas for improvement and develop strategies for enhancing patient care.
- You will be responsible for building and maintaining relationships with healthcare providers, community organizations, and other stakeholders to ensure that patients receive comprehensive and coordinated care.
- You will be responsible for staying up-to-date with the latest developments in healthcare and community health, and will be expected to apply this knowledge to improve patient outcomes and enhance the quality of care.
About You
We are looking for a highly skilled and experienced healthcare professional who is passionate about providing high-quality patient care in the community. You will have a strong background in healthcare, with a minimum of 5 years of experience in a related field. You will be a strong communicator and team player, with excellent interpersonal and problem-solving skills.
- You will have a strong understanding of healthcare policy and regulations, and will be able to apply this knowledge to ensure compliance with relevant laws and standards.
- You will have experience working in a community health setting, and will be familiar with the challenges and opportunities of providing care in this context.
- You will be able to work independently and as part of a team, and will be able to prioritize tasks and manage multiple projects simultaneously.
- You will have excellent communication and interpersonal skills, and will be able to build and maintain relationships with healthcare providers, community organizations, and other stakeholders.
- You will be able to stay up-to-date with the latest developments in healthcare and community health, and will be able to apply this knowledge to improve patient outcomes and enhance the quality of care.
