We are seeking a highly skilled and compassionate Certified Geriatric Nurse Specialist to join our team at Promedis24 GmbH. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our patients.
Key Responsibilities:- Assess and develop individualized care plans for geriatric patients, focusing on their unique needs and goals.
- Provide comprehensive geriatric care, including medication management, wound care, and mobility assistance.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes, maintaining accurate and detailed records.
- Participate in ongoing education and training to stay current with best practices in geriatric care.
- Current certification as a Geriatric Nurse Specialist (GNS) or equivalent.
