Role Overview: Your Contribution to Patient Care

As a vital member of our team, you will engage in patient support and management during their transition from hospital to home. Your responsibilities will include:

• Providing guidance, information, and education to patients and their families regarding continence, stoma care, and nutrition.
• Coordinating comprehensive post-hospital care, which encompasses product selection, prescription management, and necessary documentation within our customer system.
• Building and maintaining relationships with healthcare facilities and practitioners while fostering new client connections.
• Utilizing an iPad to manage patient aid orders, generate visit reports, and maintain nursing documentation.
• Conducting training sessions for patients and healthcare staff on product usage and general care practices.

Qualifications: What You Bring to the Team

• A completed qualification as a registered health and nursing professional or nursing specialist, complemented by at least five years of relevant experience.
• Desirable practical experience in patient care, particularly in continence or stoma management.
• Experience in fieldwork or outpatient care is a plus, though not essential.
• Ability to work autonomously and a passion for self-organization.
• A valid driver's license and residence in the relevant region would be advantageous.
