About the Role

We are seeking a skilled and experienced healthcare professional to join our team at PubliCare as an Ambulatory Care Specialist. In this role, you will be responsible for providing patient education and support in the community, working closely with healthcare providers and other stakeholders to ensure seamless care transitions.

  • You will be responsible for conducting patient assessments, developing care plans, and providing education and support to patients and their families in the community.
  • You will work closely with healthcare providers to ensure that patients receive the necessary care and support in the community, and will be responsible for coordinating care transitions between healthcare settings.
  • You will be responsible for maintaining accurate records and reporting on patient outcomes, and will work closely with the healthcare team to identify areas for improvement and develop strategies for enhancing patient care.
  • You will be responsible for building and maintaining relationships with healthcare providers, community organizations, and other stakeholders to ensure that patients receive comprehensive and coordinated care.
  • You will be responsible for staying up-to-date with the latest developments in healthcare and community health, and will participate in ongoing education and training to enhance your skills and knowledge.

About You

We are looking for a highly skilled and experienced healthcare professional who is passionate about providing high-quality patient care in the community. You will have a strong background in healthcare, with a minimum of 5 years of experience in a related field. You will be a strong communicator and team player, with excellent interpersonal and problem-solving skills.

  • You will have a strong understanding of healthcare policy and regulations, and will be able to navigate complex healthcare systems.
  • You will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
