Job Description

We are seeking a skilled and compassionate Ambulatory Care Specialist to join our team at KWA Hanns-Seidel-Haus. As a key member of our care team, you will be responsible for providing exceptional care to our residents, ensuring their comfort, safety, and dignity.

Key Responsibilities:
  • Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
  • Provide hands-on care, including bathing, dressing, and grooming, as well as assistance with daily living activities.
  • Monitor and report any changes in a resident's condition, ensuring timely interventions and adjustments to their care plan.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document resident interactions, care provided, and any notable events or concerns.
Requirements:
  • Completed training as an Altenpfleger/in or Gesundheits- und Krankenpfleger/in.
  • Experience in geriatric care, with a focus on ambulatory care.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
  • Ability to lift, transfer, and position residents with care and precision.
  • Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
