About the Job

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team on a per diem basis. As a Home Health LPN, you will provide high-quality nursing care to patients in their homes, promoting independence and improving their overall health and well-being.

Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on prevention, education, and rehabilitation.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication, promoting a patient-centered approach.
  • Document patient progress, outcomes, and any changes in their condition, maintaining accurate and up-to-date records.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or vocational nursing school.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
