This is a fixed-term post of 6 months. Please note that salary is Pro Rata for part-time posts.

The shift pattern is 5 days over 7.

An exciting opportunity has arisen for a B6 to work within an interdisciplinary Community Rehabilitation Team providing rehabilitation to patients in their own home.

The post involves working as an autonomous practitioner within a multi-disciplinary team to complete comprehensive holistic MDT and discipline specific assessments. You will be responsible for planning treatment and care and supporting hospital discharges and admission avoidance referrals.

The postholder should have excellent communication skills and be able to liaise with patient, carers, team members and other agencies to coordinate and support the patient through the service.

Experience in Adult Rehabilitation, Neurology, Orthopaedics, Medicine or Care of the Elderly Specialties is desired. A full UK driving license is essential and Community Experience would be beneficial.
