Nurse Specialist

Fresenius Medical Care is seeking a skilled Registered Nurse to join our Home Therapies team. As a Home Therapies Registered Nurse, you will play a critical role in managing patient care in an independent home therapy program. Your primary responsibilities will include providing education and support to patients and caretakers who have chosen to dialyze in the comfort of their home.
Key responsibilities:- Conduct initial visits to patients' homes to assess home therapy readiness and provide training and follow-up visits within the clinic environment.
- Participate in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
- Identify and evaluate candidates for the Home Therapies Program, including assessment of patients' home environment if needed.
- Perform assessment and identifies barriers of the Home Dialysis candidate's home environment and caretaker readiness and ability to perform dialysis treatments in the home.
- Admit new patients to the Home Therapies Program using the appropriate processes.
- Coordinate patient care, including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients.
- Assess and manages patients' response to home dialysis training and treatment therapy by following prescribed predetermined protocols.
- Provide initial and ongoing education to patient and family on home dialysis treatment and the operation and maintenance of all home therapies equipment.
- Ensure patient awareness related to transplant and treatment modality options.
- Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness.
- Initiates or assist with emergency response measures.
- Completes routine patient evaluations within the clinic setting.
- Ensures correct laboratory collection, processing, and shipping procedures are performed.
- Collaborates with the Interdisciplinary Team on the patient care plan.
- Makes referrals to Social Worker and Registered Dietitian as appropriate.
- Completes CAP requirements to either maintain or advance within the program.
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet the practice requirements in the state in which he or she is employed.
- Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
- Experience as a Registered Nurse (preferred but not required).
- Chronic/home hemodialysis experience (preferred but not required).
- Successfully pass the Ishihara Color Blind Test.
