Nurse Specialist

Fresenius Medical Care is seeking a skilled Registered Nurse to join our Home Therapies team. As a Home Therapies Registered Nurse, you will play a critical role in managing patient care in an independent home therapy program. Your primary responsibility will be to provide education and support to patients and their caregivers who have chosen to dialyze in the comfort of their own homes.
Key responsibilities:- Conduct initial visits to patients' homes to assess their readiness for home therapy
- Provide ongoing education and training to patients and their caregivers on home dialysis treatment and equipment operation
- Coordinate patient care, including assessment, planning, intervention, and evaluation for assigned patients
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Ensure patients are aware of transplant and treatment modality options
- Administer medications as prescribed or in accordance with approved protocols
- Initiate or assist with emergency response measures as needed
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which they are employed
- Experience as a Registered Nurse, preferably in a nephrology setting
- Chronic or home hemodialysis experience
