Key Responsibilities- Assess patient knowledge of diet and kidney disease, and provide education to patients and their families on the relationship between diet and kidney disease.
- Develop and implement individualized plans of care for patients with kidney disease, including nutrition assessments and counseling.
- Collaborate with the interdisciplinary team to develop and implement treatment plans that address the nutritional needs of patients with kidney disease.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal nutritional outcomes.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Registered Dietitian (RD) certification
- Minimum 1 year of experience in clinical nutrition, preferably in renal nutrition
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Basic computer skills
