Job Summary

Fresenius Medical Care is seeking a highly skilled Master Social Worker to join our team. As a Social Worker, you will play a vital role in providing psychosocial services to patients undergoing dialysis, promoting positive adjustment, rehabilitation, and improved quality of life.

Key Responsibilities
  • Assess patients' psychosocial status, strengths, and areas of need to develop effective care plans.
  • Collaborate with the interdisciplinary team to provide education, support, and counseling to patients and their families.
  • Develop and implement social work-focused interventions to address barriers to treatment adherence and improve quality of life.
  • Participate in quality review meetings and contribute to process improvement activities.
  • Provide ongoing education to patients and families regarding psychosocial issues related to End Stage Renal Disease (ESRD) and available support services.
Requirements
  • Masters in Social Work (MSW) degree.
  • State required license.
  • 2-5 years' related experience.
