Key Responsibilities- Assess patients' psychosocial status, strengths, and areas of need to develop effective care plans.
- Collaborate with the interdisciplinary team to identify interventions that address barriers and meet patient treatment goals.
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Offer supportive counseling services to patients, as permitted within the scope of their clinical training and state license.
- Assist patients with referral to community resources, such as home health services and vocational rehabilitation, to facilitate optimal treatment outcomes.
- Participate in monthly Quality review meetings with the interdisciplinary team to discuss quality indicators related to adherence, quality of life, and service recovery.
- Collaborate with the team on appropriate Quality Assessment and Improvement (QAI) activities.
- Masters in Social Work degree.
- State required license.
- 2-5 years' related experience.
