Renal Social Worker

Fresenius Medical Care is seeking a skilled Social Worker to join our team. As a key member of our interdisciplinary team, you will provide psychosocial services to patients undergoing dialysis, promoting positive adjustment, rehabilitation, and improved quality of life.
Key Responsibilities- Patient Assessment and Care Planning: Conduct comprehensive assessments of patients' psychosocial status, strengths, and areas of need, collaborating with the healthcare team to develop effective care plans.
- Psychosocial Support and Education: Provide supportive counseling services, patient education, and goal-directed counseling to patients and their families, addressing barriers to treatment and promoting optimal psychosocial status and quality of life.
- Quality Improvement and Patient Safety: Participate in quality review meetings, reporting on quality indicators related to adherence, quality of life, and service recovery, and contribute to process improvement activities.
- Insurance and Financial Assistance: Collaborate with insurance coordinators to address issues related to insurance, providing information and education to patients about payment options and facilitating access to community resources.
- Staff Support and Development: Provide training to staff on psychosocial topics, contribute to weekly team huddles, and discuss urgent patient issues with staff.
- Masters in Social Work (MSW) degree
- State required license
- 2-5 years' related experience
