Palliative Care Specialist

About the Role:

We are seeking a highly skilled and compassionate Palliative Care Specialist to join our interdisciplinary team at EVIM - Evangelischer Verein für Innere Mission in Nassau. As a Palliative Care Specialist, you will work closely with patients, families, and healthcare professionals to provide comprehensive and person-centered care.

Key Responsibilities:

  • Provide palliative care services to patients in their homes, focusing on symptom management, pain control, and quality of life improvement.
  • Collaborate with healthcare teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
  • Conduct regular home visits to assess patient needs, provide education, and support family members.
  • Participate in interprofessional teams to discuss patient cases, share knowledge, and coordinate care.
  • Develop and maintain relationships with community resources, healthcare providers, and other stakeholders to ensure seamless care transitions.

Requirements:

  • Current licensure as a Registered Nurse (RN) or equivalent in Palliative Care.
  • Minimum 2 years of experience in palliative care, with a strong focus on symptom management and patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong problem-solving and critical thinking skills, with the ability to navigate complex healthcare systems.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual respect.
