Pharmaceutical Care Specialist in the Field for Lower Saxony

For our patient-oriented home care team, with its headquarters in Hamburg, we are currently looking for a:
Pharmaceutical Care Specialist in the Field (m/f/d) in full-time (40 hours/week) for Lower Saxony.
Responsibilities- Responsible consultation, guidance, and care of patients and their relatives during and after the transition from the clinic to home and the outpatient sector.
- Care of patients according to medical prescriptions with a focus on enteral and parenteral nutrition, anti-infectives, and PCA.
- Independent and self-responsible customer care.
- Active development and expansion of customer relationships and networks, as well as deepening existing customer contacts.
- Implementation and planning of the customer acquisition process; care and communication with all parties involved in the care.
- Ensuring a call readiness.
- Examined nurse or nurse in the field of health and care or a comparable qualification.
- Further education in the fields of oncology, palliative care, and nutrition is desirable.
- Several years of professional experience are advantageous.
- Independent and goal-oriented working style.
- Enjoyment of working with patients.
- Well-developed communicative and organizational skills, team spirit, high service orientation, and willingness to engage.
- Driving license class B.
- A varied and self-determined field of activity with good development and design possibilities.
