Patient Care Coordinator

Ihre Aufgaben:
Verantwortung für die effektive Fallsteuerung und Optimierung von Prozessen zur Gewährleistung einer angemessenen Patientenversorgung im Rahmen des Entlassmanagements. Unterstützung der Stationen bei der Einhaltung der Verweildauer und Sicherstellung der erforderlichen Dokumentation. Planung, Durchführung und Koordination von fallbezogenen Leistungen unter Berücksichtigung medizinischer, pflegerischer, sozialer und ökonomischer Aspekte. Enge Zusammenarbeit mit dem Sozialdienst und der familiären Pflege zur optimalen Entlassung und poststationären Versorgung. Moderation der interdisziplinären Blitzvisite.Ihr Profil:
Abgeschlossene Ausbildung als Gesundheits- und Krankenpfleger (m/w/d) oder als Medizinische Fachangestellte (m/w/d). Idealerweise Weiterbildung oder Studium zum Case Manager (m/w/d) oder die Bereitschaft, diese zu absolvieren. Gute Kenntnisse im Sozialrecht und der Krankenhausfinanzierung. Erfahrung in der Prozessanalyse und fundiertes Wissen über die Organisation eines Klinikums. Sicherer Umgang mit EDV-Systemen. Ausgeprägtes Kommunikations- und Organisationsgeschick sowie eine zielorientierte und eigenverantwortliche Arbeitsweise. Freude an der Arbeit im interdisziplinären Team und Teamgeist.Ihre Vorteile:
