Job Summary:

Rocket match powered by notificAI GmbH is seeking a highly skilled and experienced Ambulatory Care Manager to join our team. As a key member of our organization, you will be responsible for overseeing the day-to-day operations of our ambulatory care services.

Key Responsibilities:

  • Develop and implement effective care plans to ensure high-quality patient outcomes

  • Collaborate with healthcare professionals to provide comprehensive care services

  • Manage and supervise a team of healthcare professionals to ensure efficient and effective care delivery

  • Develop and maintain relationships with healthcare providers and community organizations to ensure seamless care transitions

  • Monitor and evaluate the quality of care services to ensure compliance with regulatory requirements and industry standards

  • Develop and implement policies and procedures to ensure effective care delivery and patient satisfaction

Requirements:

  • Master's degree in Healthcare Administration or a related field

  • Minimum 5 years of experience in ambulatory care management

  • Proven track record of improving patient outcomes and reducing healthcare costs

  • Excellent leadership and communication skills

  • Ability to work in a fast-paced environment and prioritize multiple tasks
