POSITION SUMMARY:
The Outpatient Registered Nurse (RN) in the Clinical Advancement Program (CAP) is responsible for delivering and coordinating high-quality nursing care for patients undergoing hemodialysis treatment. This role may serve as an entry point into the CAP for qualified new hires or as a progression from RN CAP 1. The RN will ensure comprehensive patient care through assessment, planning, intervention, and evaluation, while also delegating tasks to support staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, the RN will engage in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient outcomes and operational efficiency.
KEY RESPONSIBILITIES:
The following duties are to be performed in compliance with Fresenius Kidney Care policies, nursing standards, and relevant regulations:
- Execute essential nursing functions under the supervision of the Team Leader, with guidance from Clinical Educators or in collaboration with fellow Registered Nurses.
- Conduct thorough and systematic assessments of patient data before, during, and after hemodialysis treatments, documenting findings in the medical record and adjusting treatment plans as necessary.
- Identify and document patient and family educational needs to provide ongoing education throughout the treatment process.
- Oversee and facilitate the daily delivery of safe and effective hemodialysis therapy in collaboration with patient care staff.
- Administer prescribed medications and document their effectiveness in accordance with established protocols.
- Participate in emergency response efforts as required.
- Act as a mentor and resource for new and existing staff, exemplifying professional behavior and work ethics.
- Ensure proper laboratory procedures are followed and manage any missed or inadequate collections.
- Collaborate with the Interdisciplinary Team to document and update nursing assessments and care plans.
- Educate patients about transplant and treatment options.
- Engage in educational and quality improvement initiatives as directed.
- May serve as a Preceptor for new staff members.
- Complete CAP requirements for career advancement.
- Perform additional duties as assigned by the Supervisor.
WORK ENVIRONMENT:
The physical demands and work environment characteristics are typical for a healthcare setting:
- This role involves direct patient care, including heavy lifting and assisting with patient mobility.
- Frequent periods of standing are required, along with the ability to bend and move equipment.
- Occasionally, the employee may need to assist in moving equipment weighing up to 200 lbs.
- The work environment includes temperature-controlled areas with moderate noise levels.
- Exposure to infectious materials may occur.
- Daily tasks involve desk work, computer usage, and interaction with patients and healthcare staff.
- Travel to training sites or other facilities may be necessary.
SUPERVISORY RESPONSIBILITIES:
The RN may oversee Patient Care Technicians, LVN/LPNs, and RNs as a Team Leader or designated Nurse in Charge, provided they meet the necessary training and experience requirements.
EDUCATION AND LICENSURE:
- Graduate from an accredited School of Nursing.
- Hold current state licensure.
- Current CPR BLS Certification.
- Must comply with state practice requirements.
EXPERIENCE AND SKILLS:
- Minimum of 2 years of experience in Nephrology Nursing is preferred.
