  1. Purpose and Scope
  2. The Hemodialysis Nurse Specialist will be responsible for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients undergoing hemodialysis treatment at Fresenius Medical Care facilities.
  3. This position involves delegation of appropriate tasks to direct patient care staff, including registered nurses, licensed vocational nurses, and patient care technicians, to ensure seamless delivery of care.
  4. The ideal candidate will have strong clinical skills and be able to work effectively in a fast-paced environment, prioritizing patient needs and collaborating with the interdisciplinary team to achieve optimal patient outcomes.

Principal Duties and Responsibilities

  1. Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other registered nurses.
  2. Collects and analyzes patient data pre, during, and post-hemodialysis treatment, documenting findings and making adjustments to the treatment plan as indicated.
  3. Assesses and documents patient/family learning needs, providing initial and ongoing education to patients and family members.
  4. Directs and provides safe and effective delivery of hemodialysis therapy to assigned patients, collaborating with direct and ancillary patient care staff as needed.
  5. Administers medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness.
  6. Initiates or assists with emergency response measures as necessary.
  7. Serves as a resource for the healthcare team, participating in staff training and orientation of new employees as assigned.
  8. Ensures correct laboratory collection, processing, and shipping procedures are performed, rescheduling missed or insufficient laboratory collections.
  9. Identifies expected outcomes, documents, and updates the nursing assessment and plan of care for assigned patients through collaboration with the interdisciplinary team.
  10. Ensures patient awareness related to transplant and treatment modality options.
  11. Required to complete CAP requirements to advance.
  12. Performs all other duties as assigned by the Supervisor.

Physical Demands and Working Conditions

  1. The position requires direct patient care, involving heavy lifting, moving patients, and assisting with ambulation, with equipment aids and/or coworkers providing assistance as needed.
  2. Frequent, prolonged periods of standing are required, with the ability to bend as necessary.
  3. The employee may occasionally be required to move, with assistance, machines and equipment weighing up to 200 lbs. and lift chemical and water solutions weighing up to 30 lbs. as high as 5 feet.
  4. The work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels.
  5. May be exposed to infectious and contagious diseases/materials.
  6. Day-to-day work includes desk work, computer work, interaction with patients, facility/hospital staff, and physicians.
  7. The position may require travel to training sites or other facilities.
  8. May be asked to provide essential functions of this position in other locations with the same physical demands and working conditions as described above.
  9. Employees are required to take the Ishihara's Color Blindness test as a condition of employment, with consideration for reasonable accommodation.
