Job Title: Ambulatory Care Manager

We are seeking a highly skilled and experienced Ambulatory Care Manager to join our team at rocket match powered by notificAI GmbH. As an Ambulatory Care Manager, you will be responsible for leading a team of healthcare professionals in providing high-quality patient care in an ambulatory setting.

Key Responsibilities:

  • Develop and implement effective care plans to ensure optimal patient outcomes

  • Lead and manage a team of healthcare professionals to provide high-quality patient care

  • Collaborate with other healthcare professionals to ensure seamless care transitions

Requirements:
