- We are seeking a highly skilled and experienced Social Worker:in or Pflegefachkraft to join our team as a Case Manager:in - Family Support Specialist.
- The successful candidate will be responsible for providing comprehensive support and guidance to families with children and youth who require intensive care and support in Berlin.
- This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
- Provide in-depth counseling and support to families with children and youth who require intensive care and support in Berlin.
- Collaborate closely with the Senate for Education, Youth and Family, other project partners, youth and social welfare offices, and care coordination centers.
- Inform families about the Berlin care network and provide social law counseling.
- Support families in understanding their child's illness and in managing daily and illness-related tasks.
- A degree in Social Work or a related field, or a degree in a related field with several years of experience in child and youth care.
- Completion of the Case Management training (DGCC) or willingness to complete it within a short period.
- Excellent knowledge of social law and experience in counseling families.
- Strong knowledge of the Berlin care landscape, particularly in the family sector.
- Interest in counseling the target group and enthusiasm for networking and coordination tasks.
- Ability to work independently, reliability, and strong communication skills.
- Valid driver's license (Class B).
