About the Role

  • We are seeking a highly skilled and experienced Social Worker:in or Pflegefachkraft to join our team as a Case Manager:in - Family Support Specialist.
  • The successful candidate will be responsible for providing comprehensive support and guidance to families with children and youth who require intensive care and support in Berlin.
  • Key responsibilities include:
  • Providing expert advice and support to families in need, including case management and coordination of services.
  • Collaborating with government agencies, healthcare providers, and other stakeholders to ensure seamless care and support.
  • Developing and implementing individualized care plans to meet the unique needs of each family.
  • Conducting home visits and providing on-site support to families as needed.
  • Participating in regular team meetings and case conferences to ensure effective communication and coordination.
Requirements

  • Master's degree in Social Work or related field, or equivalent experience as a Pflegefachkraft.
  • Proven experience in case management, child and youth care, and family support.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to develop and implement effective care plans.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality services.
  • Fluency in German, with excellent written and verbal communication skills.
