**Job Summary**

We are seeking a motivated and experienced Diabetes Care Specialist to join our team. As a Diabetes Care Specialist, you will be responsible for developing and implementing a comprehensive diabetes treatment plan, providing education and support to patients with diabetes, and collaborating with our medical team to ensure seamless care.

**Key Responsibilities**

• Develop and implement a comprehensive diabetes treatment plan, including medication management and lifestyle counseling.
• Provide education and support to patients with diabetes, including insulin pump management, glucose monitoring, and carbohydrate counting.
• Collaborate with our medical team to ensure seamless care and coordination of patient services.
• Organize and lead diabetes education programs and support groups for patients and their families.

**Requirements**

• Must be a certified Diabetes Care Specialist (DCS) with experience in diabetes education and management.
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
• Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
• Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
