**Job Summary**
We are seeking a highly motivated and experienced Diabetes Care Specialist to join our team. As a Diabetes Care Specialist, you will be responsible for developing and implementing a comprehensive diabetes management plan, providing education and support to patients with diabetes, and collaborating with our medical team to ensure seamless care.
**Key Responsibilities**
- Develop and implement a comprehensive diabetes management plan, including regular monitoring and adjustment of treatment plans as needed.
- Provide education and support to patients with diabetes, including training on insulin pumps, glucose monitoring systems, and other diabetes management tools.
- Collaborate with our medical team to ensure seamless care and coordination of patient services.
- Organize and lead diabetes education sessions and support groups for patients and their families.
**Requirements**
- Must be a certified Diabetes Care Specialist with a strong background in diabetes management and education.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
