Care Coordinator for Children with Diabetes

We are seeking a highly skilled and compassionate individual to join our team as an Integrations Specialist/Facilitator in Childcare. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities- Provide individualized support to a child with diabetes, aged 3.5 years, to ensure their health and well-being needs are met.
- Monitor and record the child's blood glucose levels, and administer insulin as required.
- Work collaboratively with the child's family and other healthcare professionals to develop and implement a care plan that meets the child's needs.
- Provide education and support to the child's family on diabetes management and care.
- Collaborate with the childcare team to ensure the child's needs are met and their care plan is implemented.
- Participate in regular team meetings to discuss the child's progress and any challenges that may arise.
- Relevant qualifications and experience in a related field, such as childcare, nursing, or social work.
- Strong knowledge of diabetes management and care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
