**Duties**:
- Provide professional case management and health planning services.
- Represent and /or serve as an advocate for the program, patients, their families in obtaining services to support their special needs.
- Provide assessment, disease management, education. Training, and other clinically based activities to coordinate care among providers, clients, and the community.
- Make adequate contact with clients with a focus on behavioral goal setting and member centered support planning.
- Collaborate with other health care providers to review actual and proposed medical care services against established coverage guidelines review criteria.
- Skill in communicating effectively with health providers as well as with network provider in developing plans of care following clinical practice guidelines.
- Maintains communication with health providers as well as network provider to ensure care is appropriate to the status of the patient and in keeping with the clinical practice guidelines.
- Examine clinical programs information to identify members for specific case management and or disease management activities or interventions by using established screening criteria.
- Working closely with clinical support staff, nursing leadership, physicians, and the population health department to achieve system goals of improving clinical outcomes for patients with chronic diseases.
- Developing and implementing individualized care plans for patients with chronic medical conditions.

- One year trial/probationary period may be required.

- This position requires Temporary Duty (TDY) or business travel approximately 30% of the time away from your permanent duty station
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.

- **Basic Requirement for Nurse (Administrative)**:
**Degree**: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.)
- **Licensure**:A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
- **Foreign Degree**: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
- **Exemption**: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
- Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
- Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

**Professional Nursing Experience**:One or more full years of specialized - professional nursing experience which includes implementing clinical research or studies, instructing and teaching individuals and/or groups regarding direct population health improvement, and disease management functions to include continuous improvement of quality patient care.
- This definition of professional nursing experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).
