**Duties**:
- Identifies opportunities for improvement in patient, provider, and system structures and processes of service delivery or self as interfaces with clinical practice guidelines.
- Performs population health improvement and disease management functions, including continuous improvement of quality patient care for BMEDDAC's assigned patient population.
- Assum responsibility and accountability for reporting compliance with clinical practice guidelines and NCQA HEDIS measures.
- When needed, compliance reporting is presented directly to local and higher levels, including Region and MEDCOM.
- Directly assists clinic providers in establishing effective and efficient practice patterns using outcome measures and benchmarking data with national standards.
- Promote and advocates for patient needs, mainly where gaps in service exist or physical, mental, or psychosocial disabilities prevent individual/family access to care and services.
- Maintain communication between all team members to ensure care is appropriate to the patient's status and in keeping with the clinical practice guideline recommendations, goals of disease management, and the organizational mission goals.
- Participate in in-services and educational offerings regarding medical and utilization management principles and practices.
- Meet with other administrative staff to develop action plans for various administrative supportive factors that affect outcomes and performance according to clinic practice guidelines.
- Plays a crucial role in developing and providing resources for training material and actively assists in teaching providers and other healthcare team members about disease management, HEDIS, and CPG measures.
- Attend and organizes meetings and seminars to educate staff and promote improvements in adherence to performance measures.
- Conclude and recommends improving fiscal outcomes and other issues affecting health care delivery for high-risk, high-cost patients.
- Conduct data reviews in specific patient aggregates that may be special concerning the Utilization Management program (UM).
- The incumbent will oversee and direct population health and disease management programs for the US Army Medical Department Activity Bavaria (BMEDDAC).
- Drafts and revises local policies related to Population Health program needs utilizing the six core principles of population health management as a framework for providing effective population-based health programs for the enrolled population.
- Document and tracks information required to conduct disease management/patient population review and analysis.
- Responsible to keep abreast of accreditation and credentialing standards prescriptive for health care services.

- Male applicants born after December 31, 1959 must complete a Pre-Employment Certification Statement for Selective Service Registration.
- You will be required to provide proof of U.S. Citizenship.
- One year trial/probationary period may be required.
- Applicants must comply with the Exceptional Family Member Program requirements as outlined in the DoD Instruction 1315.19.
- This position requires Temporary Duty (TDY) or business travel approximately 30% of the time away from your permanent duty station
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.

**Basic Requirement for Nurse (Administrative)**:
**Degree**: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.)
**Licensure**:A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
**Foreign Degree**: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
**Exemption**: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.
