Nurse (Clinical/case Management)

Duties:
- Provide professional case management and health planning services.
- Provide oversight for professional development and/or healthcare training
- Direct a case managed patient population
- Act as Nurse Consultant through holistic approach to address health care needs in collaboration with all health team members to include the patient and the patient's family as warranted.
- Represent and /or serve as an advocate for the program, patients, their families in obtaining services to support their special needs.
- Introduce innovative nursing techniques, practices, in collaboration with health care providers to identify, assess, educate, plan.
- Coordinate care through programs designed to provide efficient, comprehensive and/or cost effective service for case managed patients.
- Participate in the identification of processes, systems, practice metrics; to determine measures of care outcomes for the population served.
Requirements:
Conditions of Employment:
- One year trial/probationary period may be required.
- Male applicants born after December 31, 1959, must complete a Pre-Employment Certification Statement for Selective Service Registration.
- You will be required to provide proof of U.S. Citizenship.
- Applicants must comply with the Exceptional Family Member Program requirements as outlined in the DoD Instruction
- This position requires Basic Life Support (BLS) certification from, or in accordance with, the American Heart Association (AHA) in accordance with Army Regulation 406
- This position requires the completion of a preemployment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position.
- This position requires shift work (rotating shift).
- This position requires Temporary Duty (TDY) or business travel approximately 5% of the time away from your permanent duty station
- A Driver's license will be required.
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
- This position has specific initial background investigation and periodic reverification requirements since its duties require interaction with children and youth under the age of eighteen (18) years of age on a recurring basis.

Basic Requirement for Nurse (Clinical / Case Management):
Degree:
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S.
Department of Education at the time the degree was obtained.(

Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.

Foreign Degree:A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below.

- Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
- Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
- Professional Nursing Experience: One or more full years of specialized
- professional nursing experience which includes: 1); providing assessment, planning, implementation, coordination, evaluation, and monitoring of patients for health options and services; 2) conducting a comprehensive assessment of beneficiary's health needs in order to develop a plan of care; 3) having professional knowledge of nursing theory, hospital administrative operations and procedures and how they interact with clinical and administrative information systems;
