Our community care service IC Home 24 cares for people in need of intensive care, ventilated and non-ventilated, with a high level of professionalism and social competence. Over 600 employees at 20 locations in Germany work with us.

Onboarding und Integration neuer Klienten

Verordnungsmanagement und Antragswesen für Patienten

Patientenaufnahme inkl. Erstbesuch, Besichtigungen, Überleitung, Aufnahme, Mietvertrag, Pflegevertrag

Organisation von Providern für z.B. Nahrung, Beatmung und notwendigen Hilfsmitteln

Begleitung des MD bei Einstufung des Pflegegrads und der AKI-Begutachtung

Überwachung und Steuerung des Pflegeprozesses

SIS/Maßnahmenpläne

Umsetzung von Expertenstandards

Durchführung von Pflegevisiten

Durchführung von Gesprächen mit Angehörigen, Ärzten, Therapeuten usw.

Planung von Krankenhausaufenthalten, Arztbesuchen, therapeutische Behandlungen

Begleitung/Betreuung aller akuten AZ-Verschlechterungen d. Klienten

Begleitungen von Prüfungen von z.B. MD, Heimaufsicht oder AirLiquide

Steuerung bei Ende der Pflege des Klienten

Umsetzung der Anforderungen (z.B. Hygiene) und gesetzlichen Vorschriften

Personalmanagement

Personalführung- und Entwicklung (z.B. Jahresmitarbeitergespräche, Entwicklungsgespräche)

Kommunikation von Vorgaben, Änderungen und Informationen an Mitarbeiter z.B. im Rahmen von Teambesprechungen, Fallkonferenzen oder PDL Meetings

Personal- Dienstplanung über Medifox (inkl. Monatliche Dienstplanung, Überwachung der Einhaltung des Pflegeschlüssels, Urlaubsplanung, Ausfallmanagement, Einsatz/Planung/Dokumentation Zeitarbeit)

Stundenabrechnung pro Mitarbeiter inkl. Meldung von Krankmeldungen, Vertragsänderungen und Austritten

Koordination von Hauswirtschaft, Reinigungspersonal und Pädagogik

Personalsuche (Sichtung der Unterlagen, Terminierung der Gespräche + Hospitationen, Bewerbungsgespräche, Einstellungs- und Vertragsgespräche)

Planung/Organisation und ggf. Durchführung innerbetrieblicher Schulungs- und Fortbildungsmaßnahmen

Planung und Koordination der Ausbildung in Zusammenarbeit mit Bildungsträgern

Betreuung/ Begleitung des Onboarding- und Offboardingprozesses

Unternehmerische und administrative Tätigkeiten:

Zuarbeit Abrechnung: Abgleich der SIS/Maßnahmenplanung mit Leistungsnachweisen sowie Kontrolle der Leistungsnachweise

Beziehungsmanagement (z.B. Behörden, KKM, CM, QM, HR)

Dashboard-Pflege

Rechnungskontrolle und -freigabe

Mitwirkung im QM-Team

Qualifikation

Abgeschlossene dreijährige Berufsausbildung zur Gesundheits- und Krankenpfleger/-in oder Altenpfleger/-in oder

Mindestanforderung: Pflegefachkraft für außerklinische Beatmung

Weiterbildung: Pflegedienstleitung (min. 460 Stunden) oder Studium im Bereich Pflegemanagement

Medifox-Kenntnisse (wünschenswert)

Wünschenswert: Pflegeexperte/in für außerklinische Beatmungs- und Intensivpflege oder Fachweiterbildung Anästhesie und Intensivmedizin
