Key Responsibilities:
  • Develop and implement individualized support plans in collaboration with clients, families, and healthcare professionals.
  • Provide social support and guidance to clients, promoting their autonomy and self-esteem.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support.
  • Conduct assessments and evaluations to identify client needs and develop effective support strategies.
  • Document client progress and outcomes, ensuring accurate and timely reporting.
Requirements:
  • Relevant degree or diploma in social work, psychology, or a related field.
  • Experience working with individuals with mental health conditions and addiction.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
