About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. In this role, you will be responsible for providing high-quality patient care and support to individuals and their families in need of hospice services.
Key Responsibilities:
- Assess patients' physical, functional, psychosocial, and environmental needs, and develop individualized care plans to meet their unique needs.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Provide emotional support and education to patients and their families, promoting independence and self-advocacy.
- Document patient care and progress accurately and thoroughly, adhering to Community Hospice standards and regulations.
- Supervise and mentor hospice aides to ensure high-quality care delivery.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care and support.
Requirements:
- Current RN licensure and 1+ year of hospice or medical-surgical experience.
- Strong assessment and communication skills, with ability to work independently and as part of a team.
- Knowledge of hospice care principles, policies, and regulations.
- Ability to work flexible hours, including evenings and weekends, as needed.
