KWA Hanns-Seidel-Haus is seeking a Certified Nursing Professional (m/w/d) for full-time or part-time roles in home care services.

KEY RESPONSIBILITIES:

• Overseeing the care process with a focus on resident or client needs through effective planning and evaluation.
• Administering care in accordance with the latest nursing standards and regulatory guidelines.
• Ensuring the satisfaction and well-being of care recipients.
• Contributing to the development, execution, and enhancement of the care strategy.
• Participating in care assessments and ensuring the accuracy of evaluations.
• Assisting in MDK evaluations to determine care requirements.
• Accompanying healthcare providers on home visits.
• Managing documentation systems while ensuring effective communication of information and observations.

CANDIDATE PROFILE:

• Completed qualifications as a geriatric nurse or health and nursing professional.
• Familiarity with case management.
• Experience in planning and managing care processes.
• Proficient in standard IT applications.
• Commitment to ongoing professional development.
• Dependable and client-focused approach with empathy towards residents and their families.
• Strong teamwork, communication skills, and personal dedication.
• High level of responsibility and adaptability.
• Willingness to work in a rotating shift schedule, including weekends.
